Baystate Health, a nationally recognized leader in healthcare quality and safety, is looking for a full-time Physical Therapist to join our outpatient team in Springfield, MA.

DEPARTMENT DESCRIPTION:

SCHEDULE:

  • Full-time 40 hours
  • Clinic hours are M-Th 7a-7p and Fri 7a-5p. Will cover 2 later shifts a week

LOCATION:

  • Outpatient Rehab
  • 360 Birnie Avenue, Springfield, MA

QUALIFICATIONS:

  • Bachelor’s degree in Physical Therapy (Doctorate preferred)
  • Massachusetts Physical Therapist Licensure
  • Experienced and new grads welcome to apply
  • American Heart Association BLS CPR

RESPONSIBILITIES:

  1. Evaluates all assigned patients, develops plan of treatment, provides assessment and assists in discharge planning as appropriate for each discipline.
  2. Applies treatment plan to patients within established procedures and time frames, as determined by each discipline.
  3. Completes necessary documentation to comply with professional standards department procedures for initial consult, progress notes, clinical conference notes, and dictate reports.
  4. Provides information and consultation to patients, families, caregivers, and other health care personnel involved with patient management.
  5. Completes department operational procedures, including scheduling, billing, equipment usage, time-off requests and caseload coverage assignments.
  6. Assist with on-the-job orientation and training for student interns.
  7. Demonstrates relevant knowledge and application of clinical skills in evaluation, treatment and counseling.
  8. Meets productivity expectations as determined by the supervisor.
  9. Attend in-service training, outside seminars and other meetings as directed.
