Position Title

Physical Therapist

Reports To

Clinical Director / Rehab Director

Position Summary

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Sign‑On Bonus

$15,000

Responsibilities
  • Assess and evaluate therapeutic, rehabilitative, and functional status and participate in the development of the total plan of care.
  • Asses for muscle strength, mobility, gait, ROM potential for rehabilitation.
  • Direct physical therapy treatment.
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report to physician (or other authorized licensed independent practitioner) on patient’s reaction to treatment or changes in condition.
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy, including participation in case conferences.
  • May train patient in the use of prosthetic device.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on attainment of goals.
  • Prepare and submit a clinical progress summary as directed by Organization policy.
  • Provide physical therapy consultation to home families/caregivers when indicated.
Communication
  • Provide in-service education programs for nursing organization personnel as needed.
  • Participate in peer consultation process.
Additional Duties
  • Supervise Physical Therapy Assistants according to organization policy and state regulations.
  • Instruct, supervise, and evaluate home health aide care provided every two (2) weeks when therapy is the only skilled service.
Qualifications
  • Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapist within the State, if applicable.
  • Two years of appropriate experience as a physical therapist; community/home health experience preferred.
Skills Required
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements.
  • Pulls, pushes, and/or lifts 15‑20 pounds daily; pulls/pushes 125‑200 pounds regularly; pulls/pushes 200+ pounds regularly.
  • Sits, stands, stoops, squats, walks, and moves intermittently during work hours.
  • Drives in own car to and from patients’ homes; works in patient(s) homes as well as agency office.
  • Ability to cope with the mental and emotional stress of the position.
