Job summary

Healthcare Plus is seeking a licensed Physical Therapist to provide direct care under a physician’s plan of care. The role includes guiding patients through therapeutic exercises, gait and transfer training, and prosthetic training.

Responsibilities include documenting visits, using modalities, and educating patients on devices while collaborating with the Home Care Team and participating in in-service education.

Qualifications

• Diplomate Degree in Physical Therapy required.
• Current license as required by state law.
• CPR certification (BLS) required.
• Licensed driver with insured vehicle.

Responsibilities

• Provide direct physical therapy per physician’s plan of care.
• Treat patients to relieve pain, restore function, maintain performance.
• Guide therapeutic exercises, gait training, transfers, and prosthetic training.
• Use physical agents (heat, cold, water, light, electricity, ultrasound, massage).
• Observe and report patient responses and condition changes.
• Instruct patients on use of wheelchairs, braces, crutches, canes, prosthetic/orthotic devices.
• Maintain clinical notes and records within prescribed timeframes.
• Participate in Home Care Team meetings and in-service education.

Education

Diplomate Degree in Physical Therapy

Current state license

CPR certification (BLS)

JOB DESCRIPTION SUMMARY

The physical therapy provides physical therapy services according to the physician's plan of care

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Provides direct physical therapy in accordance with the physician's plan of care.
2. Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
3. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
4. Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
5. Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in the patient's condition.
6. Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
7. Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient's clinical record in prescribed time frames.
8. Participates in Organization Home Care Team Members meetings and Organization In-service Education Program.

POSITION QUALIFICATIONS

1. With a Diplomate Degree in Physical Therapy.
2. Current certification or license as required by state law.
3. Possesses and maintains current CPR certification.
4. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working.
