Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for per diem or part-time home health visits in the Allentown area. Join a multidisciplinary team delivering rehabilitative care to adults in senior living communities.

One year of PT experience is required. You will document care and coordinate with nursing to maintain patient independence and safety, with competitive benefits and a supportive work environment.

Qualifications

• A current Pennsylvania Physical Therapist license (PT).
• Minimum of one year of recent PT clinical experience.
• Graduation from an AP TA/AC E A A A A A accredited program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, transfer status, balance, and gait assessments.
• Implement protective interventions to prevent wound formation and promote wound repair.
• Develop and implement individualized care plans including manual exercises and gait training.
• Assess and revise the PT care plan and collaborate with nursing in multidisciplinary care.
• Educate clients and families to promote health, safety, and independence.
• Document observations and interventions in the EMR.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a per diem or part-time opportunity performing home health visits for oursenior living office. This office services adult and geriatric clients on a per visit basis in senior living communities throughout the Allentown area

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current Pennsylvania Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$65.00 - $75.00 per point
