BAYADA Home Health Care is seeking an experienced Physical Therapist for home health visits in senior living facilities. This is a full-time or part-time role serving adult and geriatric clients, with a focus on Med A/B visits.

You will join a multidisciplinary team delivering rehabilitative care and skilled nursing, helping clients remain at home while receiving quality therapies.

Qualifications

• Current New Jersey Physical Therapist license.
• Graduation from an APTA-approved program or CMAA accreditation as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, range of joints, balance, and strength.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, and balance activities.
• Continually assess and revise the PT care plan in coordination with nursing.
• Educate clients and families in rehabilitative care to promote health and independence.
• Document observations, interventions, and evaluations in EMR using a touch pad tablet.

Job description

BAYADA at Inspira is currently seeking an experienced Physical Therapist, for a full-time or part-time opportunity performing home health visits with adult and geriatric clients for our Senior Living Facilities office. This position will be a combination of Med A and Med B visits in senior living communities.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications

• A current New Jersey Physical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information

Base Pay, depending upon qualifications and subject to negotiation:

$60.00-$65.00 per point ($93,602-$101,402 annual earning potential)
