Job summary

BAYADA at Inspira Home Health Care is seeking an experienced Physical Therapist for a full-time home health role in Cumberland County, NJ, serving clients across Cumberland County with per-visit scheduling. You will be part of a multi-disciplinary team delivering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include home visits, diagnostic testing, care plan development, and close collaboration with nursing staff.

Qualifications

• NJ PT license required.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program or CMA/AAHEA-equivalent with transcript or diploma.

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance).
• Implement wound care and protective interventions.
• Develop and implement individualized care plans (therex, gait training, balance).
• Continually assess and revise PT care plans with the team.
• Educate clients and families on rehabilitative care and activities.
• Accurately document interventions and evaluations using EMR tablet.

BAYADA at Inspira Home Health is currently seeking an experienced Physical Therapist, for a full-time opportunity performing home health visits for our Cumberland County office, located in Millville, NJ. This office services clients on a per visit basis in territories throughout Cumberland County, NJ.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation: $64.00-$74.00 per point ($99,842-$115,442 annual earning potential)
