Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Savage, MN, including private residences and senior living settings. The role emphasizes autonomy, a flexible schedule, and the potential integration of telehealth within the Geriatric House Calls™ model.

Responsibilities include evaluations, individualized plans of care, and evidence-based treatments, with documentation in FOX’s EMR and collaboration with an interdisciplinary team of PTs,

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in homes and facilities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions to support mobility and independence.
• Document accurately in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and fitness staff.
• Educate patients and caregivers to promote safety and aging in place.

Physical Therapist – Savage, MN

FOX Rehabilitation is growing in Savage, MN and is looking for licensed Physical Therapists to provide one‑on‑one care to older adults in private residences, senior living communities, and other settings through our Geriatric House Calls™ model. The role offers autonomy, flexibility, and the freedom to create and manage your own treatment schedule while building lasting relationships. Telehealth may also be incorporated into the schedule.

Responsibilities

• Provide one‑on‑one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care)
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth or virtual care when appropriate to extend access and flexibility

• Part‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits eligibility based on minimum hours)
• PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation)

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
