Job summary

FOX Rehabilitation is growing in Northwood, IA and we are looking for licensed Physical Therapists to deliver one-on-one care to older adults in private homes and senior living communities.

You’ll develop individualized treatment plans, perform evaluations, and deliver evidence-based therapy to improve mobility and independence, with autonomy to manage your schedule and build lasting patient relationships. Telehealth options may be incorporated for flexibility.

Qualifications

• Must hold a valid Physical Therapy license in state of practice or be eligible to obtain
• Degree from an accredited Physical Therapy program is required
• Basic computer skills and EMR proficiency including documentation

Responsibilities

• Provide one-on-one PT to older adults in home or senior living settings
• Develop and implement individualized plans of care
• Deliver evidence-based treatment to improve mobility and independence
• Document care in the EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs and Fitness Specialists
• Educate patients and caregivers to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25-50 lbs occasionally, 10-25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
