Job summary

Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities in San Antonio, TX. You will provide evidence-based physical therapy, develop rehab programs, and contribute to resident recovery and quality of life alongside a clinically driven team.

The role welcomes therapists at all experience levels and requires graduation from an accredited university with a valid state license (or in process).

Qualifications

• Must have graduated from an accredited University and have a valid (or in process) License in appropriate state.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
