Aspire Home Healthcare of Ohio in Dublin, OH is seeking a part-time Physical Therapist to deliver one-on-one home health care, evaluate patients, and develop individualized treatment plans in patients' homes.

You'll collaborate with nurses and therapists, ensure HIPAA compliance, and take advantage of flexible scheduling, mileage reimbursement, and opportunities for growth across multiple states.

Qualifications

• Active OH PT license required for practice.
• CPR certification and TB test within last 12 months preferred.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patient condition and establish measurable treatment goals.
• Develop and implement individualized treatment plans.
• Supervise and coordinate care with PTAs and HHAs.
• Document patient progress, updates, and treatment notes accurately and on time.
• Educate patients and families on exercises, mobility, and use of adaptive equipment.
• Evaluate home environments and recommend modifications to improve safety and independence.
• Ensure all services meet HIPAA standards for confidentiality.
• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care.

Your Role:

The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.
