Job summary

Avir Vista is seeking a Physical Therapist to evaluate and treat residents and patients with physical disabilities, aiming to relieve pain, restore function, and promote healing under physician orders. The PT will apply exercise, heat, ultrasound, and other modalities while following federal guidelines.

The role includes patient evaluation, treatment planning, and collaboration with care teams to ensure accurate record-keeping and compliant documentation across shifts and settings.

Qualifications

• Bachelor, Master, or Doctorate degree in Physical Therapy.
• Current license in Physical Therapy (if required by state law).
• State-approved certification to provide advanced level treatments where applicable.

Responsibilities

• Screen and evaluate residents/patients and develop appropriate care plans under physician orders.
• Instruct residents, families, and caregivers in the use of adaptive equipment such as wheelchairs, walkers, canes and splints.
• Maintain patient records in an accurate and timely manner.
• Devises special tools and treatments for the specific needs of patients.
• Complete required forms and documents in accordance with company policy and state and/or federal regulations.
