A leading healthcare facility is seeking a Physical Therapist to evaluate and treat patients while supervising assistants and aiding in training. Physical Therapist (Remote) - Sherwood Oaks Post Acute (Healthcare)
Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral. If unable to complete evaluation within 24 hours, documentation in the medical record must clearly define the cause for delay. Develop an effective treatment plan and obtain approval for services from the referring physician. Treat patients according to the physician treatment plan.
Assist the nursing department with training Restorative Aides. Supervise Physical Therapy Assistants in direct patient care and patient-related activities, following state practice acts. Communicate with supervisors and other health team members regarding patient progress, problems, and plans. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
Participate in in-service training programs for other staff in the facility. Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation as required by PT Board State Practice Act, governmental, and third-party payer requirements. Record treatment changes according to policies and procedures.
Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Participate in discharge planning. Recommend and facilitate the ordering of necessary durable medical equipment (e.g., braces, walkers, canes, elevated toilet seats, grab bars) to promote patient independence in mobility.
Report any issues with department equipment to ensure proper maintenance. Demonstrate the ability to relate positively and effectively with patients/residents, families, staff, and professional colleagues.
Supervisory Requirements: Supervise PT assistants, aides, and students.
Qualifications:
Physical Demands: Must be able to stand/walk frequently, sit occasionally, reach, push/pull, talk/hear, and smell/taste frequently. Capable of exerting significant force, climbing, balancing, stooping, kneeling, crouching, or crawling occasionally. Must have good vision and focus ability.
Work Environment: Typically low to moderate noise levels. Reasonable accommodations available for individuals with disabilities.
