Job Description

The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

  • A licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois.
  • The position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement.
  • Serves as a role model for all therapists and clinical interns in all aspects of professional, clinical, communication, education, and leadership activities.
Job Details

This is a Full Time, salaried position at 40 hours per week for our Mobile OP PT service line, providing outpatient physical therapy at patients' homes. Shifts may vary between mornings and evenings based on patient needs, with flexibility. The role can be hybrid, primarily in-home, with options for telehealth or in-clinic care depending on census and needs. Coverage will be mainly in the surrounding area.

Responsibilities
  • Demonstrates NM values of teamwork, patients first, integrity, and excellence.
  • Maintains a friendly, professional demeanor and appearance.
  • Provides accurate, timely information to patients and addresses concerns promptly.
  • Uses effective service recovery skills for problem-solving.
  • Maintains responsibility for patient care plans through collaboration with PTs and PTAs.
  • Communicates effectively, respects diverse opinions, and contributes to team efforts.
  • Uses resources efficiently and manages work schedules effectively.
  • Participates in continuous improvement, cost reduction, and service growth efforts.
  • Adapts treatment to special circumstances, demonstrating creative problem solving.
  • Performs evaluations and re-evaluations per department guidelines, adjusting treatment plans accordingly.
  • Maintains documentation in electronic medical records, including daily notes and discharge summaries.
  • Provides patient education tailored to individual understanding and motivation.
  • Engages in departmental committees, projects, and educational efforts.
  • Builds professional relationships with healthcare team members and participates in marketing efforts.
  • Fosters respectful communication and accepts constructive feedback.
  • Completes non-patient care responsibilities timely and contributes to educational activities.
  • Supports student clinical programs and maintains productivity standards.
  • Willing to flex schedule or assignments as needed to meet patient and department needs.
Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in Illinois.
  • BLS certification from the American Heart Association within 90 days of hire.

Preferred:

  • Masters or Doctoral Degree in Physical Therapy.
