• Develop effective treatment plans and obtain approval for services.
• Communicate regularly with rehabilitation team and nursing staff.
• Record evaluations and daily treatment notes as per policy.
• Instruct patient/resident’s families in maintenance program.
• Secure necessary durable medical equipment for independence.
