Job summary

Sanford Health is seeking a Physical Therapist to plan and deliver rehabilitation services across post-acute, long-term care and home health settings. You will assess patients, develop therapeutic plans, and work to restore mobility and reduce pain in a patient-centered approach.

Responsibilities include conducting evaluations, documenting progress, educating patients and families on treatment plans and exercises, and coordinating care with a multidisciplinary team.

Qualifications

• Must be licensed as a Physical Therapist in state(s) of practice.
• Valid driver's license may be required.
• BLS certification required within 60 days of employment.
• Experience in post-acute, long-term care, or home health is a plus.

Responsibilities

• Plan, organize and deliver physical therapy programs to improve mobility and reduce pain.
• Conduct examinations, evaluations, and interventions for clients/patients/residents with injuries or diseases.
• Educate clients/patients/residents and families on treatment plans and home exercises.
• Possibly supervise assistants, aides, and students as they provide care.
• Document clinical information in accordance with departmental guidelines.
