Job summary

Sanford Health is seeking a licensed Physical Therapist to plan and deliver comprehensive therapy programs across the care continuum. The role involves evaluating patients, designing interventions, and documenting progress within regulatory guidelines.

You will educate patients and families, tailor treatment plans, and collaborate with a multidisciplinary team to optimize mobility and reduce pain in line with facility standards.

Qualifications

• Must be licensed as a Physical Therapist in the practicing state(s).
• Basic Life Support (BLS) certification required or within first 60 days of employment.

Responsibilities

• Plan, organize and deliver physical therapy programs to improve mobility, relieve pain and increase strength.
• Conduct examinations, evaluations, and interventions for clients/patients/residents affected by injury or disease.
• Document patient information, establish treatment plans and modify as needed.
• Educate patients and families about treatment plans and home exercises; ensure compliance with departmental guidelines.
