Qualifications

• Licensed PT with active state license and CPR certification.
• Minimum 2 years of clinical physical therapy experience in rehab settings.
• Proficiency in evaluating mobility, pain, and functional goals with measurable milestones.
• Ability to create individualized rehab plans and adjust based on progress data.
• Strong documentation skills with electronic medical records (EMR).
• Experience educating patients and families on home programs.
• Collaboration with multidisciplinary team and care coordination for optimal outcomes.
• Commitment to continuing education and professional growth.

Responsibilities
