Physical Therapist (PT) with Hope Home Care

Join Hope Home Care as a Physical Therapist (PT) and make a positive impact in patients' lives. This full-time position offers a competitive salary range of $90,000 to $110,000 per year, based on experience.

Location

Dayton, Miamisburg, Huber Heights, and Springfield.

Work Schedule

Monday to Friday, 8/9 AM to 4/5 PM (flexible).

Summary

The PT evaluates and assesses patient needs, formulates treatment plans, provides therapy services, oversees assistants, and collaborates with the care team to ensure quality patient care.

Responsibilities
  • Develop and implement treatment plans in consultation with the care team and physicians.
  • Perform assessments and document all activities properly.
  • Administer treatments as ordered and oversee therapy assistants.
  • Evaluate treatment effects and adjust plans as needed.
  • Complete discharge planning and coordinate follow-up care.
  • Participate in weekly case conferences and communicate effectively across disciplines.
Qualifications
  • Minimum of 2 years of physical therapy experience, preferably in acute or post-acute care.
  • Active state licensure in physical therapy.
  • Proof of current CPR certification, valid driver’s license, and auto insurance.
