Title: Physical Therapist (PT)
Job Type: PRN (Weekends)
Setting: Skilled Nursing Facility (SNF) / Long-Term Care

About Us:

Pennypack Nursing and Rehabilitation Center, is a dedicated skilled nursing and rehabilitation facility. We pride ourselves on delivering personalized and compassionate care to help our residents regain independence and improve their quality of life. We are currently seeking a licensed Physical Therapist to join our caring and collaborative therapy team.

Position Summary:

As a Physical Therapist, you’ll assess, plan, and implement therapy treatments for residents recovering from illness, injury, or surgery. Your goal is to enhance mobility, reduce pain, and support functional independence within a supportive long-term care setting.

Key Responsibilities:
  • Evaluate residents to determine appropriate physical therapy needs and goals

  • Develop and implement individualized treatment plans

  • Deliver hands-on physical therapy services in accordance with physician orders

  • Track and document residents' progress and adjust treatment plans as necessary

  • Collaborate with nursing staff, occupational and speech therapists, and families to coordinate care

  • Educate residents and caregivers on home exercise programs and mobility techniques

  • Ensure compliance with all state and federal regulatory guidelines

Qualifications:
  • Current Physical Therapist license in Pennsylvania (Required)

  • Degree in Physical Therapy from an accredited program

  • Experience in skilled nursing or long-term care preferred, but not required

  • Strong communication, clinical, and organizational skills

  • Familiarity with EMR systems and documentation standards
