Job summary

Qualifications

Position Overview: The Physical Therapist provides direct rehabilitation care in a skilled nursing facility setting, focusing on long-term care and short-term rehabilitation for patients with diverse orthopedic, neurological, and cardiopulmonary needs. Working within an interdisciplinary team, the therapist helps patients achieve functional goals and improve quality of life in a patient-centered environment.

Responsibilities include: Following treatment plans, assessing and modifying goals, supervising therapy staff, documenting appropriately, and utilizing therapy software. The role requires excellent customer service, effective communication, and adherence to safety and confidentiality standards.
