Job summary

Personal Touch Home Care seeks a licensed Physical Therapist to serve the Southside communities including Virginia Beach, Norfolk, Chesapeake, Suffolk and Portsmouth on a full-time basis. A sign-on bonus and extensive benefits are offered.

The role requires evaluating patients in their homes and developing therapy plans in collaboration with physicians. You will perform initial assessments, ongoing visits, and documentation, coordinating with the office to ensure seamless patient care.

Qualifications

• Virginia licensure required or eligible for licensure.
• Masters in Physical Therapy preferred.
• Experience in home health care is a plus.

Responsibilities

• Provide Physical Therapy services per physician's plan of care.
• Complete initial assessments and routine visits.
• Document patient information and progress for office records.
• Assess muscle strength, mobility, gait and ROM for rehabilitation.
• Instruct patients and caregivers in use of therapeutic devices.
• Coordinate discharge planning and referrals as needed.
• Supervise aides when applicable and assist in case conferences.

We are seeking a qualified FT Physical Therapist to service the Southside areas to include Virginia Beach, Norfolk, Chesapeake, Suffolk, Portsmouth and outer areas.

Job Summary

The Physical Therapist is responsible for the implementation of standards of care for physical therapy services. Completing initial assessments, routine visits and communicating all patient related information back to the office and documenting accordingly.

Job Responsibilities

• Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but will not be limited to:
• Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care. Evaluates home environment.
• Assesses for muscle strength, mobility, gait, ROM—potential for rehabilitation.
• Directing physical therapy treatment.
• Instructing patients and families/caregivers in the use and care of therapeutic appliances.
• Determining priority needs for physical therapy.
• Reporting to physician patient’s reaction to treatment or changes in condition.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with the patient, as well as instructing them to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Supervising physical therapy assistant, home health aides/personal care aides when such personnel are participating in patient’s rehabilitation regime.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary. 6. Prepares and submits clinical and progress summaries based on the attainment of goals.

Qualifications

• Currently licensed as a Physical Therapist in the State of Virginia.
• Possesses a degree from a baccalaureate or master’s program in physical therapy approved by The American

Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American

Medical Association or The Council on Medical Education of the American Medical Association.
