Job summary

Personal Touch Home Care of Virginia Inc. is seeking a qualified Full-Time Physical Therapist to serve Southside Virginia Beach, Norfolk, Chesapeake, Suffolk, Portsmouth, and surrounding areas.

The PT will implement standards of care, conduct initial assessments, and coordinate with the office on patient progress.

Qualifications

• Licensed Physical Therapist in Virginia (state license)
• Master's degree in Physical Therapy or higher from an accredited program
• Preferred two years of PT experience; home health preferred
• CPR certification required
• Strong verbal and written communication and organizational skills
• Good computer skills; familiarity with NetSmart preferred

Responsibilities

• Provide PT services per physician's plan of care.
• Assess and evaluate therapeutic status and home environment.
• Direct PT treatment and educate patients and families.
• Identify needs for other home health services and refer as appropriate.
• Prepare clinical and progress summaries and participate in discharge planning.
• Supervise PT aides and home health staff as needed.

We are seeking a qualified FT Physical Therapist to service the Southside areas to include Virginia Beach, Norfolk, Chesapeake, Suffolk, Portsmouth and outer areas.

Job Summary:

The Physical Therapist is responsible for the implementation of standards of care for physical therapy services. Completing initial assessments, routine visits and communicating all patient related information back to the office and documenting accordingly.

Job Responsibilities:

• Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:
  a. Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care. Evaluates home environment.
  b. Assesses for muscle strength, mobility, gait, ROM - potential for rehabilitation.
  c. Directing physical therapy treatment.
  d. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
  e. Determining priority needs for physical therapy.
  f. Reporting to physician patient's reaction to treatment or changes in condition.

• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with the patient, as well as instructing them to the goals of the physical therapy program for the patient by participating in case conferences.

• May train patient in the use of prosthetic device.

• Supervising physical therapy assistant, home health aides/personal care aides when such personnel are participating in patient's rehabilitation regime.

• Identifies patient and family/caregiver needs for other home health services and refers as necessary. 6. Prepares and submits clinical and progress summaries based on the attainment of goals.

• Participates in discharge planning for patient.

• Prepares and submits a clinical progress summary based on the attainment of goals.

• Provides physical therapy consultation to families/caregivers when indicated.

• Provides in-service education for nursing personnel as needed.

• Participates in peer consultation process.

• Other duties as assigned.

Qualifications:

• Currently licensed as a Physical Therapist in the State of Virginia.

• Possesses a degree from a baccalaureate or master's program in physical therapy approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association or The Council on Medical Education of the American Medical Association.

• Preferred two (2) years of experience as a physical therapist. Home Health experience preferred.

• CPR Certification required

• Demonstrates good verbal and written communication and organization skills.

• Strong computer skills; familiarity with NetSmart preferred.
