AW is a rapidly growing home health agency, with an innovative approach to patient centered care. Our team consists of over 500 employees serving over 1500 patients in Missouri and Illinois.

Job Summary:

An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence, therapy clinic, or living facility. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Regional Director of Therapy, Outpatient. This individual will be either PRN or Full-time at our St. Charles County Senior Living Facilities.

Responsibilities:

  1. Assesses patient functional ability and potential.
  2. Completes PT evaluation to determine potential benefit from therapy and to establish an effective plan of treatment.
  3. Develops and implements medically prescribed PT plan of treatment.
  4. Prepares written plan of treatment for physician ‘s signature, including clinical findings, goals, and rehab potential. Assures that all PT orders include specific procedures and modalities to be used with a specific emphasis on amount, frequency, and duration of treatment.
  5. Understands and interprets medical information from patient record and rest of interdisciplinary team.
  6. Establishes goals and discharge plans.
  7. Interprets and adapts physician orders.
  8. Coordinates treatment plan with interdisciplinary team.
  9. Instructs and supervises PTA in provided services and evaluates performance and effectiveness and progress of services provided at least every 30 days.
  10. Instructs and supervises patient/caregiver/family member in aspects of PT to be carried out.
  11. Accurately and completely documents each visit in a timely manner.
  12. Communicates with physician and interdisciplinary team all appropriate data and changes in patient condition, complications, and additional needs.
  13. Orders and /or supervises ordering of specialized durable medical equipment.
  14. Attends and participates in case conference and agency in-service programs.
  15. Other duties as assigned.

Work Environment:

Works indoors in Agency office, residential living facilities, and patient homes. Can travel to/from patient homes.

Job Relationships:

Supervised by: Regional Director of Therapy, Outpatient

Workers Supervised: Physical Therapy Assistant and Home Health Aide as assigned

Exposure Risk:

High risk

Requirements

Qualifications:

  1. Graduate of an accredited program of Physical Therapy, culminating in a minimum of a Bachelor of Science degree;
  2. Current certification with the National Board for Certification and valid therapy license in the state of Missouri.
  3. Currently licensed in the state(s) in which practicing.
  4. Two (2) years experience, preferred.

Physical Requirements:

Ability to perform the following tasks if necessary:

  • Ability to participate in physical activity.
  • Ability to work for extended period of time while standing and being involved in physical activity.
  • Moderate lifting.
  • Ability to do extensive bending, lifting and standing on a regular basis.
