Flagstar Rehab is seeking Physical Therapists (PT) for Skilled Nursing Facilities. Therapists have the flexibility to choose assignments that fit their schedules, with options for independent work and support in setting up a corporation if desired.

Benefits include daily updates on facility needs, remote onboarding, and a work model that supports work-life balance. Compensation is competitive, with hourly rates of $46 (W2) and $57 (1099).

Requirements and Responsibilities:

• Current NYS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meet all State and Federal employment requirements for US employment in a Nursing Facility
• Provide evaluations, create treatment plans, and monitor patient progress in a SNF setting, addressing conditions like TBI, CVA, Stroke, Parkinson's, and others.
