Job summary

Lifepoint Rehabilitation is seeking a PRN Physical Therapist to join Kindred Hospital - Melbourne LTACH. You will evaluate patients, develop and adjust treatment plans, and coach nursing staff and families to support continued recovery.

You will document care, communicate progress to the team, and ensure compliance with regulatory and safety standards while maintaining a clean and safe environment.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current and unrestricted PT license in the state of practice.
• Current CPR certification.
• Strong organizational and communication skills.

Responsibilities

• Evaluate patient condition and develop a treatment plan, guiding progress day by day.
• Instruct nursing staff and family on follow-through programs that support progress.
• Communicate patient progress to supervisor and team; assist with scheduling and post charges.
• Document patient care per regulatory, licensing, payer and accrediting requirements.
• Maintain a safe, clean work area and equipment; adhere to professional standards and laws.
• Other duties as assigned.

How you'll contribute

A Physical Therapist who excels in this role:

• Put your physical therapy skills to work where they're really needed -evaluate a patient's condition, develop a treatment plan, and help them get better, day by day. You'll also instruct the nursing staff and the patient's families on follow-through programs that build on the progress they've made
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records
• Document patient care in accordance with regulatory, licensing, payer and accrediting requirements
• Instruct patient's family or nursing staff in follow-through programs
• Maintain equipment and work area in a safe and clean condition
• Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws
• Other duties as assigned
