Are you a passionate Physical Therapist looking to make a difference in the lives of older adults? Join our dedicated Home Health Rehab Team in Palos Park, IL, where we are currently seeking a Per Diem (PRN) Physical Therapist to support patients in their homes across a wide service area.

Coverage areas include: Hickory Hills, Oak Lawn, La Grange, Burr Ridge, Oak Park, Alsip, Blue Island, East Side Chicago, Lansing, Calumet City, Homewood, Country Club Hills, Tinley Park, Orland Park, Homer Glen, Lockport, Lemont, Bolingbrook, and Romeoville.

Responsibilities

As a PRN Physical Therapist (PT), you will establish appropriate goals and creatively execute the treatment plans along with supervision of the Physical Therapist Assistants. As a member of our interdisciplinary team, you will be required to maintain productivity standards and complete required documentation. More importantly, you will help your patients achieve critical outcomes while acting as a trusted partner to the community you serve.

Qualifications

• Be a graduate of an accredited school for Physical Therapists.
• Hold a valid state license.
• Ability to work flexible hours and weekends.
• It’s great to have had acute care, long-term care and/or rehabilitation experience but not always required!
