Qualifications

  • Graduation from an accredited Physical Therapist Program and active PT license in Iowa.
  • Minimum of 6 months of physical therapy experience; training new PTs is also acceptable.
  • Strong clinical judgment, interpersonal skills, dexterity, and compassion.

PRN home health assignments throughout Polk, Dallas, and Warren Counties.

As a Home Health Physical Therapist, your responsibilities include:

  1. Providing home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
  2. Collaborating with a home health team including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, focused on the patient's plan of care and goals
  3. Observing movements, conducting assessments, documenting progress, and modifying care plans as needed
  4. Assisting patients with exercises and hands-on therapy to manage pain and increase mobility
  5. Educating patients on their care plan, goals, exercises, and self-care
  6. Coaching family members on the care plan, therapy goals, and safe assistance with exercises

Must-have qualifications for Home Health Physical Therapists:

  • Graduation from an accredited Physical Therapist Program and active PT license in Iowa
  • Minimum of 6 months of physical therapy experience; training new PTs is also considered
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, interpersonal skills, dexterity, and compassion
