Job summary

Elara Caring is seeking a Physical Therapist for home health PRN in Wood and Hancock Counties, Ohio. You will evaluate patients, develop plans, and deliver in-home PT services, collaborating with the care team to ensure optimal outcomes.

Requirements include an APTA-approved PT program, current license, and at least 1 year of healthcare experience. Travel between patient homes is required with reliable transportation. Competitive benefits are offered.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and assigned locations.
• Ability to frequently lift, push, pull, and assist up to 50 pounds.

Responsibilities

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
