Vista Manor Healthcare & Rehabilitation seeks a Physical Therapist (PT) for PRN assignments in Titusville, Florida. You will assess, plan, and implement personalized therapy to restore function and improve residents’ quality of life.

Responsibilities

• Develop, implement, and evaluate individualized physical therapy treatment plans in accordance with regulations and best practices.

• Administer therapeutic interventions to restore function and reduce pain.
• Collaborate with physicians, nursing staff, and interdisciplinary team to coordinate care.
• Document evaluations, treatment plans, progress notes, and discharge summaries.

Key Responsibilities

• Develop, implement, and evaluate individualized physical therapy treatment plans in accordance with long-term care regulations and best practices.
• Perform comprehensive patient evaluations, including assessments of strength, mobility, balance, and functional abilities.
• Administer therapeutic interventions to restore function, reduce pain, and prevent further disability.
• Collaborate with physicians, nursing staff, administration, and interdisciplinary team members to coordinate comprehensive care.
• Maintain high clinical standards and ensure compliance with federal, state, and facility guidelines.
• Support the Physical Therapy department as needed, including collaboration on staffing and patient scheduling.
• Provide mentorship, guidance, and support to new or less experienced team members when applicable.
• Accurately document evaluations, treatment plans, progress notes, and discharge summaries in a timely manner.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited institution (Master’s or Doctorate preferred).
• Active and unrestricted Physical Therapy license in the state of Florida.
• Prior experience in a skilled nursing facility, hospital, or related healthcare setting required.
• Strong clinical assessment and communication skills.
• Ability to work effectively in a collaborative, team-oriented environment.
