Responsibilities

What you will do: Responsibilities include but are not limited to:

  • Perform evaluations and develop treatment plans
  • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
  • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
  • Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members
  • Coordinate with other disciplines to ensure collaboration, care coordination, and improve patient outcomes
Qualifications

What you will need:

  • Minimum of a bachelor’s degree in Physical Therapy from an accredited college or university
  • Current, unrestricted license as a Physical Therapist in the practicing state
  • Current Basic Life Support or Cardiopulmonary Resuscitation certification as per state regulations
  • At least one year of work experience as a physical therapist is preferred
  • Knowledge of rehabilitation techniques related to complex neurological injuries
  • Effective and professional communication skills, both verbal and written
  • Ability to lift up to 50 pounds
  • Capability to perform moving, lifting, or transferring tasks involving up to 100 pounds following safety procedures
  • Physical requirements include: fine motor skills, visual acuity, walking, standing for extended periods, pushing/pulling, bending/stooping, reaching, kneeling, and repositioning
  • A health screen or examination may be required prior to and periodically during employment, based on location policies and regulations
