Job summary

Portneuf Medical Center is seeking a Physical Therapist to join our rehab team on a PRN basis. You will conduct assessments, create treatment plans, and provide direct patient care to improve strength, mobility, and function.

You will educate patients on home exercise programs, document progress, and collaborate with the rehab team to support safe discharge planning. CPR certification and a PT license are required.

Qualifications

• Education: Graduate of an accredited PT program.
• Experience: One year in rehab setting preferred.
• License: Current Physical Therapy license (PT).
• Certifications: CPR certification required.

Responsibilities

• Conduct physical therapy assessments and develop treatment plans.
• Provide direct patient care to restore strength, mobility, and function.
• Educate patients and caregivers on home exercise programs.
