Job description

Synergy Care is seeking a qualified Licensed Physical Therapist for a PRN position at an Independent Living Community in Peoria, IL. The ideal candidate will provide quality patient care and work well within a team.

Job responsibilities include:

• Providing physical therapy and related services in accordance with occupational therapy principles and Synergy Management Solutions guidelines.
• Evaluating patients, developing treatment and discharge plans, supervising assistants and aides, and participating in case management.
• Representing Synergy Management Solutions' commitment to quality and service in all interactions with the facility.
• Adhering to all state regulations regarding occupational therapy.
