Job summary

Interim Healthcare of Central, VA is seeking a Physical Therapist to provide compassionate, home-based therapy in Newport News and surrounding areas. You’ll help patients regain strength, reduce pain and reach functional goals through personalized plans of care.

As part of a home health team, you’ll assess progress, document outcomes, educate patients and families, and promote independence. Virginia PT licensure, CPR, and a valid driver's license are required; 1:1 patient ratios and flexible

Qualifications

• Graduate of an accredited Physical Therapist program with Virginia licensure.
• Current CPR certification.
• Up to 2 years PT experience, preferably in home health.
• Knowledge of home health regulations.
• Valid driver’s license and reliable transportation.
• Excellent clinical judgment and compassionate approach.

Responsibilities

• Provide home-based therapy to patients with mobility, strength and functional goals.
• Work as part of a home health team aligned with plan of care and goals.
• Observe movements, assess, document progress and modify care plan as needed.
• Assist with exercises and hands-on therapy to improve mobility and manage pain.
• Educate patients on plan of care, goals, exercises and self-care.
• Educate families on plan of care, therapy goals and safely assisting with exercises.

As a Physical Therapist, here's a big picture-view of what you'll do:

• Provide home-based therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Works as part of a home health team focused on the patient's plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercised and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Educate families on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for ourPhysical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Virginia
• Current CPR certification
• Maximum of 2 years of Physical Therapy experience, ideally in home healthcare
• Knowledge of state and federal home health regulations
• Good clinical judgment, stronginterpersonal skills, dexterity and compassion
• Valid Driver's License and reliable transportation
