Responsibilities

• Evaluate assigned patients and establish appropriate treatment goals and programs.
• Provide ongoing evaluation of patient’s status and goals, modifying treatment for timely progression.
• Develop comprehensive goals to maximize patient outcomes and facilitate return to community activities.
• Coordinate treatment with other clinical/therapeutic services and community resources. Attend clinical meetings as needed.
• Educate patients and families, develop home programs, and instruct caregivers to ensure safe and effective discharges.
• Prepare for patient discharges, including ordering equipment and recommending therapeutic services for continuity of care.
• Assess patients in home, school, work, or community environments to facilitate community reintegration, recommending adaptations as needed.
• Identify and manage levels of care, delegate tasks to PTAs and support staff to optimize outcomes and service utilization.
