Responsibilities

1. Performs physical therapy evaluation and treatment of patients based upon physician orders/referrals.
2. Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
3. Reviews patient records for current diagnosis, past medical history, precautions, and contraindications.
4. Establishes measurable goals and develops a treatment plan to move the patient toward his or her maximum level of independence and/or function.
5. Regularly re-assesses the effectiveness of the treatment plan, progress of the patient towards goals, and the need for modifications.
6. Communicates patient's needs and progress to the treatment team, physician, patient, family members, as applicable.
7. Consults with medical providers regarding patient evaluation, treatment, progress, and discharge.
8. Recommends and assists in securing adaptive equipment for patients to improve function and incorporates its use into the rehab program.
9. Maintains and submits evaluation, treatment, and billing documentation as required by regulations and company policies.
10. Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA, and applicable laws.
11. Supervises Physical Therapist Assistants and Aides in patient care activities.
12. Participates in staff development and continuing education to maintain and enhance clinical skills.

Qualifications

1. Bachelor's Degree in Physical Therapy from an accredited program.
2. Current, unrestricted Physical Therapist license in the state of operation.
3. Current BLS or CPR certification as per state regulations.
4. Preferably, at least one year of work experience as a physical therapist.
5. Knowledge of rehabilitation techniques for complex neurological injuries.
6. Effective communication skills, both verbal and written.
7. Strong organizational skills and attention to detail.
8. Physical ability to perform job duties, including sitting, lifting, standing, bending, and walking.
9. Health screening may be required prior to and periodically during employment.
