Job summary

Ambassador Healthcare at College Park in Fort Myers, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Qualifications

• Active Florida Physical Therapist license is required.
• At least 1 year of PT experience in a nursing home or skilled facility preferred.
• Strong clinical, communication, documentation, and interpersonal skills.
• Ability to work effectively within an interdisciplinary rehabilitation team.

Responsibilities

• Evaluate residents and develop individualized treatment goals and plans.
• Implement PT treatment plans and complete required documentation accurately.
• Address gait, ambulation, strength, balance, endurance, mobility, and functional independence.
• Instruct residents in safe mobility, transfers, and therapeutic activities.
• Evaluate progress and adjust treatment programs as needed.
• Recommend and train residents in adaptive equipment and devices as appropriate.
• Support collaboration with the rehab and care teams to ensure effective care.
• Participate in resident care planning and staffing conferences.
• Supervise supportive personnel per Florida regulations.
• Educate nursing and medical staff on PT techniques, mobility, safety.
• Monitor therapy equipment and supplies and communicate needs.

Ambassador Healthcare at College Park is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. The Physical Therapist will provide individualized therapy services designed to improve residents' mobility, strength, balance, coordination, endurance, independence, and overall quality of life.

Key Responsibilities

• Evaluate residents and develop individualized treatment goals and appropriate plans of care.
• Implement physical therapy treatment plans and complete all required documentation accurately and timely.
• Provide therapeutic interventions addressing gait, ambulation, muscle strength, coordination, endurance, mobility, posture, sensory awareness, sitting and standing skills, balance, and functional independence.
• Instruct residents in safe mobility, transfer, ambulation, and therapeutic activities.
• Evaluate resident progress and modify treatment programs based on individual needs and response to therapy.
• Recommend, design, fit, and train residents in the use of adaptive equipment, orthotics, prosthetics, and other assistive devices as appropriate.
• Select and apply appropriate physical therapy modalities and therapeutic techniques based on resident needs.
• Collaborate with the interdisciplinary rehabilitation and care teams to ensure appropriate and effective treatment.
• Participate in resident care planning and staffing conferences.
• Supervise supportive personnel in accordance with Florida regulations and professional standards.
• Provide education and training to nursing, medical, and other facility staff regarding physical therapy techniques, mobility, positioning, and safety.
• Monitor therapy equipment and supplies; communicate repair, replacement, and supply needs as necessary.
• Maintain a safe, caring, and compassionate environment while respecting each resident's dignity, rights, preferences, and individual needs.
• Maintain compliance with applicable state regulations, professional standards, and facility policies.

Requirements

• Current, active, and unencumbered Florida Physical Therapist (PT) license.
• At least 1 year of physical therapy experience in a nursing home or skilled nursing facility preferred.
• Strong clinical, communication, documentation, and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary rehabilitation team.
• Compassionate and resident-focused approach to care.
