Job summary

Rehab Without Walls is expanding in the Vancouver, WA area and seeking experienced Physical Therapists to join our Home & Community Program. Our model brings outpatient-level PT directly to clients in homes and community settings, supporting return-to-work and mobility goals.

We offer a PRN W2 position with flexible scheduling, hourly pay (not per visit), drive time/mileage reimbursement, and educational programs.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS/CPR certification per state regulations.
• Minimum one year PT experience preferred.
• Knowledge of rehabilitation techniques for neurological injury.
• Strong verbal and written communication skills.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and participate in meetings.
• Perform discharge evaluations and provide home safety recommendations.
• Coordinate with treatment team and family.

Rehab Without Walls is expanding in the Vancouver, WA area and seeking experienced Physical Therapists to join our Home & Community Program. Established in 1995, RWW has grown to serve clients across 21 states, delivering innovative, patient‑centered rehabilitation.

Our Home & Community model brings outpatient-level physical therapy directly to clients in homes and community settings, allowing care to happen where life actually takes place.

What you'll do: Deliver functional PT in home and community settings; support real‑world goals such as return to work, return to driving, and community mobility; collaborate with an interdisciplinary team including OTs, SLPs, and LCSWs.

Responsibilities

What you will do:

• Perform evaluations and develop treatment plans.
• Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members.
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.

Qualifications

What you will need:

• Minimum of a bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist in the state where you practice.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
• Minimum of one year's work experience as a physical therapist preferred.
• Demonstrated knowledge of rehabilitation techniques related to complex neurological injury.
• Effective and professional verbal and written communication skills.
• Ability to lift 50 pounds; lifting up to 100 pounds may be required following safety procedures.
• Fine‑motor skills, visual acuity, and walking/standing for extended periods.
• Ability to push, pull, bend, stoop, reach, kneel and frequently position patients.
• Health screen or examination may be required prior to assignment and periodically thereafter, as per local or state regulations, to verify physical capability.
