Job summary

Community Rehabilitation Hospital West, operated jointly with Lifepoint Health, is seeking a PRN Physical Therapist in Indiana. You will perform assessments, design care plans, and deliver PT interventions to restore function and reduce pain in rehab patients.

Applicants must hold a DPT, be licensed as a PT in the state, and have or pursue BLS/CPR certification; inpatient rehab experience is preferred. The team emphasizes collaboration and patient-centered care.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current licensure as a Physical Therapist in the practicing state.
• Inpatient rehab experience preferred.
• Current Basic Life Support/CPR certification.

Responsibilities

• Performs initial and ongoing assessments of patient condition.
• Establishes a plan of care aligned with identified problems and patient goals.
• Plans and carries out individually designed programs of physical treatment to improve function and relieve pain.
• Performs therapy interventions using standard PT techniques appropriate to patient age and condition.
• Directs and supervises PT assistants, students, and support staff; assesses competence and delegates tasks.

How You'll Contribute

A Physical Therapist who excels in this role:

Performs initial and ongoing assessments of patient's condition.

• Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.
• Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient.
• Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication.

What We're Looking For

A Doctorate degree from an accredited Physical Therapist program. Additional requirements include:

• Current licensure as a Physical Therapist in the state where the hospital is located
• Inpatient rehab experience preferred
• Current Basic Life Support/CPR Card
