Job summary

Lifepoint Rehabilitation, part of Lifepoint Health, is seeking a Physical Therapist for the St. Charles Health System facility in Oregon.

The PRN role focuses on inpatient rehabilitation, providing expert PT services to adult and geriatric patients in a 13-bed unit, with a strong emphasis on safety and quality outcomes. You will collaborate with a multidisciplinary team, assess and update plans of care, and contribute to patient progress and discharge planning.

Qualifications

• Current licensure as a Physical Therapist in the state where the hospital is located (Oregon).
• Inpatient rehab experience preferred.
• BLS/CPR certification currently valid.

Responsibilities

• Assumes responsibility and accountability for the care of patients by PT staff under supervision as needed.
• Assesses patients' needs and initiates appropriate physical therapy interventions.
• Observes progress and may recommend changes to the physician.
• Performs assessments and reassessments, documents QI scoring.
• Addresses pain and adjusts therapy plans to optimize function.
• Develops and updates individualized care plans for physical therapy.
• Communicates patient progress and discharge barriers in team conferences.
• Ensures patient safety and reduces healthcare errors in all care activities.
• Performs other duties as assigned.

St. Charles Health System is a 13-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve.

How You'll Contribute

• Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
• Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
• Observes and evaluates treatment affect, accomplishment of short and long term goals. Recommends change to physician, if needed.
• Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
• Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
• Formulates and updates patient specific plan of care for physical therapy
• Reports patient progress and barriers to discharge in each patient team conference
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Other duties as assigned
