Angels of Care currently has opportunities for part-time and full-time Physical Therapists (PT).

Angels of Care Pediatric Home Health is a clinician-owned and operated home health agency with experienced and knowledgeable staff serving the special needs community. Pay Range: $65-$85 per visit

Job Description

A Physical Therapist (PT) will implement treatment programs to assist pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays by planning and administering physical therapy services in the home and community.

Responsibilities
  • Provides high-quality care and meets the needs of the patient and family by performing evaluations, interpreting assessment results, developing goals appropriate to the child, and creating and implementing physical therapy treatment plans in conjunction with the physician.
  • Assists pediatric patients in developing or regaining physical, neurological, cognitive, and/or social/emotional functioning, improving their level of independence through skilled interventions to maximize each child's potential.
  • Participates in educating, coaching, and empowering caregivers and families to develop skills for carrying over therapeutic activities into the child's daily routine.
  • Coordinates with referral partners to provide services for children in accordance with physician orders.
  • Ensures the continuation of therapeutic plans after discharge by designing and instructing patients, families, and caregivers in home exercise programs.
  • Documents patient care services and care coordination in an electronic medical record system.
  • Maintains patient confidentiality at all times.
