Job summary

Sigma Health Rehab in Corning, NY is seeking a per diem Physical Therapist (PT) to join our Skilled Nursing Facility team. You will evaluate residents, design rehabilitation plans, and work with a compassionate group of therapists in a state-of-the-art rehab gym.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.

Responsibilities

• Evaluate residents' conditions and develop rehabilitation plans.
• Relieve residents' pain using exercises, hands-on therapy and stretching.
• Help residents perform tasks and monitor their progress.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidentiality and comply with privacy requirements.
• Comply with federal, state, and local regulations and professional standards.
• Collaborate with the healthcare team to support patient care.

Education

Graduate of an accredited Physical Therapy Program

Current New York State PT License

Duties of an exceptional therapist include:

• Evaluate residents conditions & develop appropriate physical rehabilitation plans

• Relieve resident’s pain using exercises, hands-on therapy & stretching

• Help residents through their tasks and monitor their performance

• Maintain resident confidence & protects facility by keeping information confidential

• Complies with federal, state, and local legal and professional requirements

PT Requirements Include:
