Job summary

Momentum at South Bay for Nursing and Rehabilitation is seeking Per-Diem Physical Therapists in New York. You will evaluate patients, collaborate with therapy teams, and deliver treatment plans per prescriptions while documenting in the electronic records system.

You should hold a New York PT license, demonstrate strong organizational and communication skills, and be able to work with patients of varying needs.

Responsibilities

• Collaborate with therapy teams and other staff
• Perform evaluations and complete all necessary documentation
• Provide recommendations supporting communications/assessments
• Deliver therapy as outlined in treatment plans and prescriptions
• Provide treatments that improve patient inabilities/challenges
• Independently manage workflow including documentation in the electronic records system
• Familiar with PDPM
• Align therapy treatments with Medicaid/Medicare billing

Responsibilities/duties:

• Work collaboratively with all therapy teams and
• Complete evaluations and all necessary documentation
• Provide recommendations supporting communications/assessments
• Provide therapy as outlined in treatment plans and prescriptions
• Offer treatments that help improve patient’s inabilities and/or challenges
• Independently manage workflow which includes allotting time for treatment documentation of individual treatments and notes in the electronic records system
• Familiar with PDPM
• Align therapy treatments and plans with an understanding of Medicaid and Medicare billing
