Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated:

Physical Therapist(PT)

Shifts Available:Full-Time, Part-Time & PRN

Service Areas with Openings:Northeast, Southwest, Pearland, and Clear Lake

Responsibilities:

  • Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
  • Implement physical therapy care plans.
  • Coordinate care with the home healthcare team.
  • Educate patients and family members about health conditions.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:

  • Physical Therapist in the state of Texas.
  • Home health experience highly preferred.
  • Knowledge of home health regulations and general healthcare compliance.
  • Experience with HCHB EMR. (preferred but not required)
  • Able to handle physical workload.
  • Good driving record.
  • Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
