Putnam Nursing & Rehabilitation Center is a 160-bed skilled nursing home and rehabilitation facility located in Putnam County, New York. We are dedicated to maintaining excellence and integrity in all aspects of our operations, providing high-quality care with respect and compassion through an interdisciplinary approach focused on personalized goals to maximize residents' potential and quality of life.

Responsibilities

• Assist in planning, organizing, developing, and implementing Physical Therapy services according to federal, state, and local standards, guidelines, and regulations.
• Plan, develop, organize, evaluate, and implement the physical therapy programs, goals, and policies to help residents achieve their highest physical, mental, and social well-being.
• Interpret and instruct in physical therapy policies and procedures to staff, residents, and families as necessary.
• Participate in discharge planning, development, and implementation of resident care plans and assessments (MDS).
• Supervise Physical Therapy Assistants as per State Physical Therapy Practice Act.
• Maintain treatment records, resident files, and progress notes, and liaise with families and residents.

Requirements

• Current license and registration to practice as a Physical Therapist in NY.
• Current malpractice insurance, license in good standing.
• Dedicated to providing compassionate care, energetic, enthusiastic, and passionate about quality care.
• Genuine interest in helping residents who are ill, injured, disabled, elderly, or emotionally upset.
• Patience, cheerful disposition, and willingness to assist residents and co-workers.
• Excellent communication, customer service, and organizational skills.
• Ability to work independently and in a team, under pressure, with flexibility.
• Covid-19 vaccinations encouraged but not mandated; vaccination is based on CDC eligibility guidance.
