Job Summary

Summary

Outpatient Neuro PT Experience Preferred

Restores Patient Physical Functioning by interpreting physical therapy evaluations and test results; determining physical therapy treatment goals and plans; administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities and in using assistant and supportive devices. They will be responsible for Physical Therapy Student and Physical Therapy Assistants as well as training new staff as needed. They will also participate in program development and research as required per site.

Essential Functions

-Establishes therapy goals and formulates a treatment plan based on evaluation results.
-Supervises PTAs according to organization policy and state regulations.
-Provides individual, family and group therapy sessions for clients engaged in outpatient therapy
-Participates in the interdisciplinary group for hospice patients and the case conference meetings for home health patients.
-Participates in discharge planning.
-Provide initial assessment of clinical needs using standardized tools and protocols and facilitate recommendations for treatment.
-Record all pertinent data in the patient’s medical records
