Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist (PT) for a contract assignment in a SNF/LTC setting. The role focuses on assessing referred patients, designing care plans, and delivering skilled PT services under physician orders.

The ideal candidate has graduated from an accredited PT program, holds an active PT license or is pursuing licensure, and maintains CPR certification.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess and modify care to meet goals of PT interventions.
• Collaborate with all disciplines to plan and evaluate team goals.

Responsibilities

• Assess referred patients that require rehabilitation services.
• Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients.
• Document and record patients' conditions.
• Educate patients and families in an appropriate physical therapy method.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
