Centra Healthcare Solutions, Inc. seeks an experienced or newly graduated Physical Therapist for a contract assignment in a SNF/LTC facility in California.

The PT will be responsible for developing the plan of care and delivering skilled PT services in accordance with physician orders, while collaborating with the care team. The role requires graduation from an accredited program, current PT licensure or active licensure application, and CPR certification.

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or in process to obtain licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.

A great SNF / LTC Facility is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment! This is a contract position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

Must have graduated from an accredited school. Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT. Current CPR certification. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services / interventions in accordance with physician orders. Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
