Job summary

Centra Healthcare Solutions Inc seeks an experienced or newly graduated Physical Therapist to join a great SNF/LTC facility for a contract assignment. This contract position offers a day schedule and accountability to the organization’s vision and goals.

Responsibilities include planning and delivering skilled PT services, building care plans, and collaborating with a multidisciplinary team to achieve patient outcomes.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

• Deliver skilled PT services consistent with physician orders.
• Assess patients, adjust plans to meet therapy goals, and collaborate with the care team.

• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
