Centra Healthcare in Columbia, SC is seeking a licensed Physical Therapist for contract work. The PT will join a SNF/LTC setting, deliver skilled therapies, develop care plans, and collaborate with the care team to meet patient goals.

The position is contract-based with day schedules and weekly pay. Must be a graduate of an accredited program and hold or be pursuing state licensure as a PT.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a PT or actively pursuing licensure.
• Develop a plan of care for each patient.
• Provide skilled PT services per physician orders.
• Assess patient needs and modify plans to meet goals.
• Collaborate with multidisciplinary team to reach outcomes.

Job Description:

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
