Overview

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

Responsibilities

• Assess referred patients that require rehabilitation services.
• Provide direct patient care to evaluate medical condition, functional capabilities, limitations and restrictions.
• Determine potential for rehabilitation and set realistic, achievable goals.
• Document and record patient condition.
• Educate patients and families in an appropriate physical therapy method.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the physician.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or be proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
